چکیده

BACKGROUND To determine factors influencing the clearance of fragments after extra-corporeal shock wave lithotripsy (ESWL) for lower pole calyceal (LPC) stones. METHODS In the period between July 1998 and Oct 2001, 100 patients with isolated lower polar calyceal calculi or= 14 years, were included in the study. Intravenous urograms (IVU) were reviewed to define the LPC anatomy (width of the infundibulum and pelvicalyceal angle). Study end points i.e. stone free status; number of shock waves used and number of sessions were correlated with variables like LPC anatomy, body mass index and stone size. RESULTS At three months follow up the clearance for stone size 90 degrees ) had stone clearance of 94 and 100% respectively. For the infundibular width of < 4 mm, the stone clearance was 93% were as for > 4 mm, it was 100%. For body mass index (BMI) less than and > 30 kg/m2, the stone clearance was 92 and 95% respectively. CONCLUSIONS There is a trend towards more ESWL sessions and shock wave requirement in patients with acute pelvi-calyceal angle and narrow infundibulum but it is not statistically significant. Size (<or= 20 mm) and BMI has no relation with stone clearance. With modern lithotripter, stones up to 20 mm could primarily be treated by ESWL, irrespective of an un-favorable lower polar calyceal anatomy and body habitus.

Comparison of Lower Pole Renal Calculi Clearance after Extracorporeal Shock Wave Lithotripsy with and without percussion, diuresis and inversion maneuver

Aims: To determine whether Percussion, diuresis and inversion (PDI) maneuver following shock wave lithotripsy (SWL) can enhance stone clearance from lower calyx as compared to SWL alone. Methods: 60 Patients with lower pole stones (8-15 mm) were randomized to SWL plus PDI (group A) or SWL alone (group B). Shock waves were delivered to all patients according to a standard protocol. Group A patie...

Extra-corporeal shock wave lithotripsy.

Extra-corporeal shock wave lithotripsy (ESWL) has proved to be a revolutionary advance in the treatment of renal stone disease. It, itself, is non-invasive but may necessitate or be used as an adjunct to more invasive auxiliary procedures. The basic principles of lithotripsy, the clinical experience thus far and probable future applications are discussed.

Therapy-resistant nephrolithiasis following renal artery coil embolization

BACKGROUND Transcatheter renal artery embolization is an effective and minimally invasive treatment option for acute renal bleeding. Early post-interventional complications include groin hematoma, incomplete embolization, coil misplacement and coil migration. Late complications are rare and mostly related to coil migration. CASE PRESENTATION A 22-year-old woman with a history of recurrent sto...

Efficacy and cost-effectiveness of extracorporeal shock wave lithotripsy for solitary lower pole renal calculi.

PURPOSE We determined the efficacy of extracorporeal shock wave lithotripsy monotherapy and compared its cost-effectiveness with percutaneous nephrolithotomy for the management of lower pole renal calculi. MATERIALS AND METHODS The efficacy (stone-free rates at 3-months posttreatment) of shock wave lithotripsy with the modified Dornier HM3* machine was determined retrospectively in 114 patien...

Salivary lithotripsy in the era of sialendoscopy

The traditional management of obstructive salivary disorders has been replaced by minimally-invasive gland-preserving techniques including shock-wave lithotripsy, sialendoscopy, interventional radiology and endoscopically video-assisted trans-oral and cervical stone retrieval, of which sialendoscopy is considered to be the method of first choice. Primary endoscopically controlled stone extracti...
